This patient portal allows you to access your patient record and to enter your pharmacy information, past medical history, skin disease history, medications, allergies, and social history. This site is your medical/clinical history.

If you do not have a user name and password or have questions about how to set up your patient portal, please contact us at 573-234-1000.

Once you enter the Portal (patient site) with the username and password provided above, you will arrive at the “Patient Data” page. On this page you will be able to review, add and/or edit your pharmacy information, as well as medical history including past medical history, skin disease history, medications, allergies, and social history. We suggest you update this information before each visit to save time at your visit.

Pharmacy: Select “Add surescripts pharmacy,” enter the name of your pharmacy and the town or zip code where the pharmacy is located and press “search.” Your pharmacy should be displayed as an option. Search by pharmacy phone number, not store number, and the pharmacy will also display. Please select your pharmacy and press the “save and continue” button at the bottom.

Past Medical History: Check off the appropriate boxes regarding your medical conditions and past surgeries (please disregard pediatric history). Press the “Save and Continue” button at the bottom.

Dermatology: Enter any history of previous dermatological conditions, and family history of melanoma; and press the “save and continue” button at the bottom to move forward.

Medications: Click “mark no medications” button if you are not taking any medications. If you are taking medications, click the “add new medication” button to enter each medication on your list. Search each medication then click it (you can add with the dose or at the top, you can pick to add with unspecified dose.) Once you have added all your medications, press the “save and continue” button.

Allergies: If you have no known drug allergies, click “Mark NKDA”. If you have drug allergies, select “add new allergy” button to enter each of your medication allergies. Please describe the reaction where possible. Once you have added any/all your new medication allergies, press the “save and continue” button at the bottom.

Social History: Complete the appropriate information and press the “save and continue” button at the bottom.
